• Collaborative approach to respond to individuals with multiple needs
• Coordinates services for families with at least one child who is involved with two or more service providers
• Program goal is to provide families access to needed services and ownership over treatment planning
• Core values include consumer involvement, unconditional care, belief in growth, learning and recovery, cultural responsiveness, family centered and strength based
• Participants must be Vernon County residents
